Accuracyofendoscopistsestimateofpolypsize:Acontinuousdilemma

摘要： AIM:Toexaminethediscrepancy,ifany,betweentheendoscopistsestimateandpathologistsmeasurementofcolonicpolypsize.METHODS:Weretrospectivelystudied88patientswhounderwentcolonoscopywithaclearunequivocaldocumentationofpolypsizebyboththeendoscopistandpathologist.Endoscopistmeasurementswerebasedonthevisualestimateofpolypsizeseenonhighdefinitionscreens.Themeasurementwasdonebyourpathologistsafterformalinfixation.Wecomparedtheendoscopistestimateofthepolypsizetothepathologistmeasurementinordertoexplorethediscordancebetweenthetworeadings.Dataregardingdemographicsandmethodofpolypectomy(snarepolypectomyvsexcisionalbiopsy)wascollected,aswell.Statisticalanalysissoftwarestatisticalsoftwarewasusedtoanalyzethedata.RESULTS:Ourcohortincluded88patientsfromwhich111polypswereremoved.Fifty-two(46.8%)ofthe111polypswereexcisedusingbiopsyforcepsandfiftynine(53.2%)wereremovedbysnare.Inthebiopsyforcepsgroup,themeanpolypsizedocumentedbythepathologistwas0.38±0.19cmandthemeanpolypsizedocumentedbytheendoscopistwas0.54±0.16cm.Themeandifferencewas0.15cm(P<0.001).Inthesnaregroup,themeanpolypsizedocumentedbythepathologistwas0.54±0.24cmandthemeanpolypsizedocumentedbytheendoscopist0.97±0.34cm.Themeandifferencewas0.42cm(P<0.001).Combiningbothgroups,themeansizedocumentedbypathologistwas0.46±0.23cmcomparedto0.76±0.35cmdocumentedbytheendoscopist.Themeandifferencewas0.3cm(95%CI:0.23-0.36).CONCLUSION:Postpolypectomymeasurementbythepathologistaregenerallysmallerthantheendoscopistsestimate. （共6頁）
