摘要: Increasingnumberofendovenoustechniquesareavailableforthetreatmentofsaphenousveinrefluxandendovenouslaserablation(EVLA)isafrequentlyusedmethod.Anewlydevelopedalternative,basedonthermaltherapy,isendovenousmicrowaveablation(EMA).Thisstudyevaluatedtheeffectofthetwoprocedures,intermsofcoagulationandhistologicalchanges,inoccludinglateralveinsingoats.Twelveanimalswererandomizedintotwogroup,with6treatedwithEMA(EMAgroup),andtherest6withEVLA(EVLAgroup).Resultsofcoagulation,includingcoagulation,fibrinolysisandplateletactivation,wereassessedatthreeorfourdifferenttimepoints:before,immediatelyafter,24h(and48h)afterablation.Thediameterchange,ameasureofefficacy,wasultrasonographicallymeasuredbeforeand1monthaftertheablation.Histologicalchangesweregrosslyandmicroscopicallyevaluatedimmediately,1and3month(s)aftertheablation.Thelengthoftheablatedveinandpreoperativeaveragediameterwerecomparablebetweenthetwogroups.InbothEMAandEVLAgroups,severalcoagulationparameters,fibrinolysisandplateletactivationparametersonlyunderwentslightchanges.UltrasoundimagingdisplayedthatthediameterreductionoftheveinstreatedbyEMAwassignificantlylargerthanbyEVLA,inconsistentwiththeresultsofmacroscopicexamination.Microscopicexaminationrevealednecrosisandthickeningoftheveinwall,andocclusionofthelumenwithin3monthsafterablationinbothEMAandEVLAgroups.ItisconcludedthatEMAisaminimallyinvasivetherapy,whichappearstobesafeandeffectivefortreatmentoflateralveinsingoats.
